You can attach a form (site survey) on a previously completed job by doing the following:

In the Navigation Menu select Schedule Jobs

Search for the customer and tap on their name - this will take you to a screen with a list of their jobs.

Tap the job that you want to attach a form to - this will take you to a screen with the job schedule information

Change the job date to today, so it will show up on your schedule again allowing you to complete the job a second time.

Go to View Schedule, tap the job and tap Arrived on the far right

Once you have arrived at the job, touch the Open Form button
